NICU experience. Job De_script_ion Position Summary: Responsible for the coordination and development of program. The nurse will work in a collaborative and collegial relationship with all members of the health care team to actively coordinate care of patients at the BIDMC and within the community. Supports building and maintaining effective patient care teams by providing guidance and situational support to nursing staff and other disciplines. Assists the program medical director with the development and implementation of program goals, _object_ives , policies and procedures. Conducts formal and informal educational programs to support patient care. Collaborates and participates with colleagues in Nursing, medicine and other health disciplines in interdisciplinary program planning and evaluation. Primary Duties and Responsibilities: 1. Assumes responsibility for assessment , planning implementation and evaluation of the plan of care for a panel of patients and/or episodic needs according to professional and organizational standards for quality and productivity Assesses total needs of patient population including pre-admission and post discharge scenarios and continuing care needs. Supports building and maintaining effective patient care teams by providing guidance and situational support to nursing staff and other disciplines. Collaborates with other health care disciplines within the medical center and community to develop effective patient care programs and to analyze cost-benefit and value. 2. Collaboration and Program Development Collaborates with colleagues in the hospital and community to develop an effective patient care program. Maximizes available resources and incorporates multi- disciplinary services into program design and implementation. Develops programs to meet patient needs and that address clinical, organizational, social, and financial issues. Collaborates with nursing and hospital administration to obtain the needed human and fiscal resources. Maintains interdisciplinary approach for planning and communicating information for appropriate patient care Demonstrates appropriate collaborative and consultative relationships with other providers within the institution and the community Participates in quality and process improvement activities to improve patient care and/or related systems Identifies a process, with his/her consulting physician, to insure that pre_script_ive practice is reviewed, according to regulatory requirements. 3. Education Identifies learning needs of patients and staff and collaborates with other team to determine priorities for educational programs. Participates in interdisciplinary education programs of the unit and provides clinical precepting for nursing students, nurse practitioner students, medical students and others. Participates in formal and informal peer review processes to improve the overall quality of nursing practice. Identifies self learning needs and updates clinical knowledge and skills through formal and informal education and review of current literature. 4. Research Participates in and supports nursing research by identification of pertinent problems for investigation. Remains aware of and utilizes current research findings for clinical practice and determines efficacy for current practice setting. 5. Leadership Provides leadership that supports hospital and departmental policies and goals, and influences change in the health care delivery system. Interprets issues related to health care and its delivery to nursing staff, patients and the community. Proposes and implements strategies to assist nursing staff with organizational change. Participates in the management of a unit or department in collaboration with the nurse mana Qualifications Qualifications 1. Licensed to practice as an advanced practice nurse in the Commonwealth of Massachusetts by the Board of Registration in Nursing or Masters preparation as a Clinical Nurse Specialist. 2. M.S.N. with ability to assume responsibility for delivery of care in the clinical setting. 3.
